Gathering information on hazardous substances and mixtures and notification of addresses of premises where drug precursors of category 2 are placed on the market belong to key tasks of the Inspector for Chemical Substances and Preparation.  In order to facilitate fulfilling the obligations imposed on the industry we prepared templates of relevant documents to be submitted to the Inspector. Please note: the templates are not official documents but represent possible forms of documents required by appropriate legal acts.
